Tax Information

Below is important information on how to access your 1095-B tax form, which shows the months in which you and your dependents had health coverage during 2025. Please note, there are no federal fines if you did not have health coverage in 2025, but some states do have their own health coverage requirements.

UNITE HERE HEALTH will not mail the 1095-B form to you, unless you lived in California, New Jersey, Rhode Island, or Washington D.C., any time in 2025 or you request a copy. If you had Kaiser Permanente coverage, your form 1095-B is available from Kaiser Permanente at www.kp.org/mytaxdocs.

To request a copy of your form:

  2. Click the link labeled 1095-B Tax Forms
  3. You can then print or save a copy of your 1095-B form
